Marital status affidavit

Hi guys,

Sorry if this post is not appropriate for this forum, but I could not find any other Laos-related forum that looks remotely alive.

So I'm going to marry a guy from Laos. The marriage registrar requires an "[marital status] affidavit signed by a witness (close relative)". Does anyone know how this is done in Laos? Of course I asked them for a clarification or a standard form that I could use, but it took them two weeks to reply to my previous email so I'm not expecting to hear from them soon.

Normally an affidavit would be a declaration signed by a notary. But a witness/close relative? Would it still need to be signed by a notary? Village/district chief maybe?

Any help would be greatly appreciated!

In Thailand affidavit must be obtained at your countries embassy.
