Hello,聽 we are planning to retire in the Algarve. What cities do you suggest we target?
We will be taking advantage of the golden visa scheme.

I also notice there is free health care provided in Portugal. Why then is everyone talking about additional insurance?

If you can afford the Golden Visa, you will want private healthcare.聽 Yes, there is public healthcare that is very cheap, but there are long waits and they are very under-staffed. Nearly everyone who can afford it, including the Portuguese, has some kind of private insurance or discount medical plan, allowing the public health waiting rooms to be filled with those who have no other choice.
