I am certain you could get a Bahrain driving license over the counter. There was another discussion on the same topic. Let me try to look that up for you.

Confirmed that you can get a Bahrain driving license over the counter without the need to go through the full testing process. The USA is on a list of countries whose licenses can be converted to Bahrain license over the counter.

I am getting mine switched from US to Bahrain this week.  The time frame they gave was about 90 minutes to get it all sorted.  I think the cost is 26 BD.

I agree with the comments above. I got mine in 20 minutes flat. I suggest you go to the traffic directorate in the evening around 5 pm. That way, you will not have to wait long. When I went, there were no people waiting. Take passport size photographs with you. Fill up a form, get your vision tested and hand over your CPR. Pay 26 BHD and voila,you have a brand new Bahraini driving license.

They will inspect the US license and return it to you. Also, I think they close at 6 pm. Just FYI.

hi there. just to check if my countries (malaysia) is in the list, do I have to convert to International license first ? Or my existing license will do ?

it looks like your ok KEL.TCY, no need to change to international, just go to traffic office, and they will tell you what to do

I'm from Malaysia. Working in Bahrain 7 year. Yes, for Malaysian no need to get international license. Just only your Malaysian license to Bahrain license with only 26 BD. But need your CPR first to do this. as I know.
