I dont understand how Wife's name would be in the Husband's passport, but anyways husband's name on the wife's passport really helps but it is not mandatory !!

Rafeek,

On Indian  and Pakistani passports, male and unmarried female names are followed by their father's names and for married females husbands name is mandatory instead of father's name. Wife's name is never mentioned on husband's passport.

Your wife will not be issued with a visitor's or a family visa until she includes husband's particulars on her passport

Husband's name in the wife's passport is enough.

My wife's passport doesnt include my name and I have a family visa. Then it must be true from case to case.

You are correct, you probably got your family visa before 2001, I think the rules have become stricter since.
