The list is a lot longer than that; the most important thing is that they must know the applicant well.聽 Not sure if a Dutch professional will do as well; the actual list is ; at the bottom is the contact details; might save you a bit of bother if you give them a call and ask if your Dutch doctor (or one of the others on the list) can sign it.
